Identification
This species is the largest of its species-complex, reaching 2.3 cm in length. Very variable coloration often with orange-yellow head and either orange or light yellow wavy lines or bands on mostly black body. Limb color is equally variable.

Range and habitat
The species is endemic to Peru and occurs in the Departamento San Martin and Departamento Loreto in north-central Peru. Lives in pre-montane primary forests and forest borders, between 300 and 700 m.

IUCN RED LIST : VULNERABLE
CITES : Appendix II
Status : The species has a small range and many populations have been heavily affected by illegal collection for the wildlife trade. Range mostly unprotected.
