Identification
Adults can reach about 3 cm in length. Different color morphs exist all with black background coloration. Presence of two broad longitudinal bands (yellow, blue or orange) on sides of body. Back is either black, yellow-spotted or fully orange.

Range and habitat
The species occurs in South America and is endemic to Peru, being found from the Andes east to Rio Huallaga. Lives in primary and secondary tropical moist forest, from 270 to 1200 m.

IUCN RED LIST : VULNERABLE
CITES : Appendix II
Status : The species has a fairly small range and it currently suffers from habitat degradation and destruction.
